Please complete this once your child's place is confirmed. It gives our leaders the medical, emergency and consent information we need to keep your child safe. All details are kept confidential and used only to run the camp safely.

Children bring a packed lunch, snacks and a drink. Water refills are available. Please send a nut-free lunch where another child has a nut allergy and we've asked.

6 Photography & social media

We sometimes take photos/video at camp for coaching, our website, social media and our YouTube channel. We only use images of your child if you opt in below, and you can change your mind at any time. See our Safeguarding Policy.

Yes — I consent to photographs/video of my child being used by RallyPoint / Suffolk Pickleball Co for coaching, promotion, the website, social media and YouTube. No — please do not use any images of my child. (Your child can still take part fully.)

7 Collection & drop-off

Children are signed in and out each day, and released only to you or the adults you name here.
